EMIS Patient Access

Patient Access is a service offered by the practice where you can request prescriptions, book appointments and view parts of your medical record online.  The service is available 24 hours per day and we encourage all patients to think about whether it would be suitable for them.

Please note that from March 2016 information available through Patient Access will be more detailed including some details of consultations and medical history.  The practice has formulated this statement of intent to notify patients of this change.  Patients who are already registered for online services will need to complete one of the consent forms below to benefit from this enhanced level of access.

To sign up you need to complete one of the forms below, the proxy form is for patients over the age of 13 who want to nominate someone else to manage their account such as next of kin or a carer.  Please read the information leaflet below then complete the relevent form and bring to the surgery along with two forms of identification (one photographic, such as driving license or passport and the other proof of address such as a utility bill).

The practice routinely writes to children aged 13 asking them to either reconsent to manage their record themselves or for their parents to manage their record on their behalf.
